Requirements vary as to the types and frequencies of Assisted Living Assessments.

Assisted Living Nurse

 

One-size does not fit all when it comes to assessment requirements for Assisted Living facilities. State requirements vary; the needs of the clientele at different facilities varies, and your corporate requirements will vary. CliniDex lets you establish different Assessment Occasions with their own subset of questions from your master list. Some of the assessment occasions you may have could include:

 

Pre-Admission Assessments
There are at least three reasons that Assisted Living facilities will conduct a pre-admission assessment.

1. Some states (like Minnesota) require that a prospective resident be assessed for appropriateness for the facility before being admitted.

2. Even where not required by regulation, most facilities have a pre-admission screening process to verify that the resident is a good fit for the Services they provide.

3. Some facilities that provide services to residents with a higher level of acuity need to ensure that all of the necessary equipment or supplies required to serve the client are provided.

 

Admission Assessments
The admission assessment is used to determine the client's current needs for service. Among other issues addressed, the admission assessment includes questions about the clients need for services relating to:

  • Activities of Daily Living (ADLs)
  • Independent Living
  • Medications used
  • Physical status
  • Orientation and cognition
  • Safety.
As with the other assessment occasions, you can customize the assessment for your campus to include or exclude questions to fit the needs of your campus.

Clinical UpdateAssisted Living Nurse
(Quarterly, Annually, or upon significant change of condition)

The Clinical Update is a follow-up to the admission assessment. This assessment is typically performed either on an annual basis or when there is a significant change of condition. The approach used in CliniDex gives you visibility to the answers provided in the admission assessment or the previous clinical update.  
 
As with the other assessment occasions, you can customize the assessment for your campus to include or exclude questions to fit the needs of your campus.
 
Supervisory Visit
 
The meaning of a "supervisory visit" varies by state. For example, in Minnesota, this is used to mean the occasion of the nurse supervising a delegated task, and providing additional training if needed. Many facilities use this occasion to also update the client's assessment.  
 
As with the other assessment occasions, you can customize the assessment for your campus to include or exclude questions to fit the needs of your campus.

Transfer/Discharge

Detailed forms about the resident's status often need to be filled out when they are transferred or discharged. CliniDex expedites this by adding several transfer/discharge questions to the CliniDex question set. When someone leaves your facility, you are able to complete a transfer or discharge form using the up-to-date assessment data in your system. This provides a complete, professional report with minimum effort.
