CliniDex Assessment Forms  Requirements vary as to the formats required for Assisted Living forms. Some facilities enter Assessments completely electronically. Others will use a paper-based approach to record the assessment details before entering the assessment into CliniDex. CliniDex offers several forms to make performing and recording an assessment via paper-based forms a simple process.  Blank Forms Blank forms list all the questions and possible responses that appear on the specified assessment. The blank form is used when no other assessment has yet been performed for that prospect or resident.
In addition to listing all the questions and possible responses that appear on the specified assessment, worksheets indicate the previously entered assessment answers. To use the worksheet, simply note any changes on the worksheet and then later update it in CliniDex.
Summaries detail the questions and responses that were chosen for the specified assessment, including any notes that were captured during the assessment process.
