CliniDex The Assessment


Assisted Living NurseThere is one master list of assessment questions, from which are chosen the questions that make up the active set of questions for a campus or facility. From this active set, each assessment occasion contains a set of questions unique to the purposes of that assessment. Questions are chosen to be included or excluded from the assessment occasions based on the needs of the facility.

Assessment occasions include Pre-Admission, Admission, Clinical Update, Supervisory Visit, and Transfer/Discharge. These occasion types can be customized to fit your assessment requirements.

When you perform an assessment, it can be entered straight into CliniDex, or you can use a paper-based process to print the corresponding form, fill in the assessment, and then enter the responses into CliniDex.

Data is retained from one assessment occasion to the next. For example, the Pre-Admission Assessment data you enter will be visible when you do the subsequent Admission Assessment and Clinical Updates. When you change an assessment response, the previous value is saved in the history, along with the start and end dates corresponding to that response selection.

All CliniDex question responses are flagged as to their relevance to subsets such as Fall Risk, Elopement Risk, Vulnerability, Supplies, Vaccinations and Vital Signs. These subsets can be summarized and reported on after the assessment is completed.

The assessment tool can be configured to automatically create a service when a specific assessment response has been selected. For example, if you determine that a resident needs escorts to meals due to cognitive or mobility issues, CliniDex can automatically create that service for you.
