ResiDex Assisted Living Software

  ResiDex provides a simple format for tracking incidents that occur within an assisted living facility or group home. The incident questions are customizable and most are answered by clicking on drop-down list options.  Incident reports and summaries are available for tracking information and analyzing data.


Incident Report Questions (Fall Example):

• What type of injury?

• Task being performed?
• Assistive device being used?
• Medical provider called?
• Family called?
• Witnesses?
• What was the explanation for the fall?
• Was 911 called?
• Was resident able to activate the emergency call system?

ResiDex allows unlimited follow-up notes to be added to a specific incident:

• Add a follow-up note any time following the incident
• Notes are saved in a history and accessible for viewing and/or printing
• Follow-up notes are categorized by time and author
