ResiDex Incident Tracking

ResiDex provides a simple format for tracking incidents that occur within an Assisted Living facility or group home. The incident questions are customizable and most are answered by clicking on drop-down list options.
Incident Report Questions (Fall Example): - Type of injury sustained in the fall
- Location of fall
- Task being performed when fall occurred
- Was assistive device being used at time of fall?
- Medical provider called?
- Family called?
- Witnesses to fall
- Explanation of fall
- Was 911 called?
- Was resident was able to activate the emergency call system correctly?
ResiDex offers several types of incident question sets according to type.  Incident Types:
- 911 Call
- Aggressive Behavior
- Elopement
- Fall
- Financial Exploitation
- Medication Error
- Other Injury
- Possible Abuse
- Possible Neglect
- Possible Theft
- Skin Breakdown
ResiDex allows an unlimited number of follow-up notes to be added to a specific incident. Incident Notes:
- Add a follow-up note any time following the incident
- Notes are saved in a history and accessible for viewing and/or printing
- Follow-up notes are categorized by time and author
